Click on Umpire Pay Sheet Form - print a copy and bring it to your games.
Enter your name and other requested information, including your signature, on the bottom of the form.
Enter all information for each game you umpire including the park name and field number.
Get one of the coaches at the game to sign the line for the game to confirm you worked the game.
When you want to start a new sheet, drop the completed sheet at the Chan Rec Center Front desk Umpires folder.
NOTE: Keep a copy of each Pay Sheet you turn in for your records !
I expect that umpires will be paid monthly based on the forms you have submitted.
